• Proof of Identity: State ID or Driver's License (Age 21+ verification).

• Referral: One (1) Professional Recommendation Letter.

• Signed Handbook: Your executed Student Acknowledgement form.

• Exit Portfolio: upload the following filled out forms: UD, RAT, FAT, Scoring Tool, MAR to the request #2.​
