THE "BIG 3" EMERGENCY NUMBERS

  1. Life-Threatening: 911

  2. Abuse/Neglect Reporting: OHCQ & Police (Report within 24 Hours) +1-401- 402-8015

  3. Poison Control: 1-800-222-1222

    (Post these next to every phone)

CRITICAL TIMELINES (The "When")

  • Resident Agreement: Signed PRIOR TO or AT admission.

  • Service Plan: Completed within 30 days of admission. Reviewed every 6 months (or if condition changes).

  • Medication Review: Delegating Nurse (DN) reviews residents on meds every 45 days.

  • Menus: Posted 1 week in advance; kept on file for 6 months.

  • Fire Drills: Conducted Quarterly on EACH shift (Day, Evening, Night).

VITAL RANGES (The "Safe Zone")

  • Food Safety "Danger Zone": 41°F – 135°F (Keep cold foods below 41°, hot foods above 135°).

  • Fridge Temp: Below 40°F.

  • Freezer Temp: 0°F or below.

  • Hot Water (Resident Taps): 100°F – 120°F (Check weekly to prevent burns).

MEDICATION SAFETY (The "5 Rights")

Before giving any pill, check these 5 things:

  1. Resident

  2. Medication

  3. Dose

  4. Time

Route (Mouth, Skin, Eye, etc.)

 

Schedule II Narcotics: Must be under double-lock and counted at every shift change.

 

R.A.C.E. (Fire Procedure)

R - RESCUE anyone in immediate danger.

A - ALARM (Pull the lever / Call 911).

C - CONTAIN (Close doors/windows to stop smoke).

E - EXTINGUISH (Only if small) or EVACUATE.

 

STAFFING RATIOS (HR)The Rule: Sufficient staff to meet the needs of residents 24/7.

Awake Overnight? Required if you have residents with Level 3 needs or specific waivers.

Delegating Nurse (DN): REQUIRED if residents need medication administration or nursing tasks.

Success Metrics:

  • You know the residents' life stories, not just their diagnoses.

  • You knock before entering and treat the suite as the resident's private home.

  • Residents are groomed according to their style, not yours.

 

STOP! DO NOT ADMIT IF...(Unless you have a specific Waiver)

They have active Tuberculosis (TB).

They require a Ventilator.

They require General Anesthesia on-site.

They are a danger to themselves or others.

The woods care (stage 3 or 4)
